Opiate free (Atkins)

1. Administer acetaminophen (intravenous [IV] or oral [PO]).
2. Administer ketorolac.
3. Administer 2 g magnesium sulfate at the start of the case and an additional 2 g at the end.
4. Perform lidocaine induction at 1 mg/kg ideal body weight (IBW), followed by an infusion calculated as 1.5 × IBW / 6 in mg/min for non-weight-based dosing on the infusion pump.
