Access: PIV x1-2
Airway: Oral ETT
Induction: Lidocaine/Propofol. Remifentanil bolus for akinesia or succinylcholine.
Maintenance: Surgeons prefer no paralytic. So, propofol/remifentanil/phenylephrine
Emergence: Awake
Antibiotics: Cefazolin
Position: Standard

Notes

Have something in line for BP, especially when surgeons ask for table down back up to see symmetry of breasts. Surgeons may stand between patient arms and head, so position poles and drapes accordingly.
